NPI Lookup Service. Search by NPI, provider name, organization, city, state or ZIP

NPI stands for National Provider Identifier. The NPI is a 10-digit identification number that is completely unique. The NPI number by itself does not contain any identifiable information such as a provider’s speciality or location. The NPI is assigned to individuals or organizacions for their lifespan and it is independent of key provider information type updates like a change of practices, location or speciality. NPI numbers are de-activated if the provider goes out of business, retires or dies. Deactivated NPIs will not be assigned to another health care provider.

The NPI number replaces the multiple identification numbers from legacy providers, which included Medicare UPINs, State Medicaid Ids, and commercial payer Ids.

This free NPI look up service allows you to search a provider’s registration information based on the provider’s name, organization name, provider’s city, state, zip or NPI.
